Your First Visit

You can expect your first appointment to take a little bit longer than a usual session. Your counselor will ask you questions about you to get to know you and your struggle: your personal journey, your goals in counseling, why you are seeking counseling, etc. There will also be forms to fill out. If you would like to fill out those forms before coming, feel free to! You will have ample time to fill them out while you are here.

Please arrive early to your first appointment!

What to Expect

We share a parking lot with Swannanoa Valley Christian Ministries (SVCM). Our building is the two story house on the right side of the parking lot. The entrance is accessible from the back of the building, where the entrance ramp begins.

Black Mountain Counseling Center is wheelchair accessible. The ramp into the building serves as the main entrance. If you have trouble with stairs, please let your counselor know so they can reserve the appropriate room ahead of time.

Our front door opens to our waiting room. Your counselor will meet you there to show you to the room.

We have a unisex restroom that is wheelchair accessible.

Please let your counselor know if you have any allergies to nuts or scents. We do occasionally have candles burning.

What to Bring

  1. Your insurance card if you have insurance and plan to use it.
    We accept many types of insurance.

  2. Paper forms if you are filling out any physical papers.

  3. Proof of income if you are enrolling in the Client Assistance Program (CAP). (This could include pay stubs, tax returns, W2 form, 1099 form, employee letter, unemployment documentation, disability insurance, court-ordered payments, or other. If you have questions about proof of income, please call our office.)

Forms

There are a number of forms to be filled out before we can receive you as a client. You have the option of bringing them in to your first session already completed, but you will have some time during the session to fill them out if you hadn’t already. You can also fill the forms out and submit them online via our HIPAA compliant client portal.

Everyone, both in-person clients and virtual clients, will need to fill out the Telebehavioral Health Informed Consent form (see form in the forms section below).

Fill Forms online

For the first time, all clients have the opportunity to fill out forms securely online through the patient portal. Your assigned counselor will create your account and send you a welcome email. Next, your counselor will send the appropriate documents to fill out and sign through the patient portal before your first appointment. 

Sign into your patient portal below. If you feel uncomfortable filling out personal information online, you can print the forms and bring them in on your first visit.
